Supported Living Coach (Rhinelander 10-20 hours/week)

Headwaters Inc. is an Equal Opportunity Employer looking for a part-time Supported Living Coach in the Rhinelander Area to work between 10-20 hours/week including one overnight shift every other week. As a Supported Living Coach, you will join a team of others who support people with disabilities in their communities and homes by providing:

  • Assistance to allow people served to live more independently and happily in their homes by working with them to achieve their personal goals, e.g. healthy eating, exercising, or doing fun activities in their home
  • Assist with day-to-day needs (transportation, cleaning, bathing, preparing meals, medical appointments, etc.)
  • Go with people served in the community to do activities (Shopping, Special Olympics, movies, etc.)
  • Provide encouragement and coaching to people who are working to develop daily living skills & healthy habits
  • Write documentation about the support you provided during your shift
  • Work with a variety of needs including medical, behavioral, and mental health supports.

  • Assistance to allow people served to live more independently and happily in their homes by working with them to achieve their personal goals, e.g. healthy eating, exercising, or doing fun activities in their home
  • Assist with day-to-day needs (transportation, cleaning, bathing, preparing meals, medical appointments, etc.)

Community Coach (Rhinelander 10-28 hours/week)

Headwaters Inc. is an Equal Opportunity Employer looking for a part-time Community Coach at our Rhinelander location in our Day Services Program to work between 10-28 hours/week. As a Community Coach, you will join a team of others who support people with disabilities attending our Day Services Program in the community by providing:

  • Access and engage in community resources and activities with participants
  • Help participants build confidence, independence, and a sense of community
  • Use exploration and discovery methods to identify and build skills and interests
  • Build community membership and help facilitate interactions and friendships
  • Help make participants life meaningful to them
  • Go with participants in the community to do activities (Shopping, Libraries, Volunteer Opportunities, bowling, movies, etc.)
  • Provide encouragement and coaching to people who are working to develop daily living skills & healthy habits
  • Write documentation about the support you provided during your shift
  • Work with a variety of needs including medical, behavioral, and mental health supports.
