Headwaters, Inc. is a not-for-profit community based work/rehabilitation center located in Rhinelander, Wisconsin, which began in 1969. We provide services to people with developmental disabilities, physical disabilities, chronically mentally ill, and people who are economically disadvantaged, and children with special needs in Forest, Oneida, and Vilas Counties. Our adult training program consists of community integration, vocational evaluations, sheltered work, case management, transportation, and community employment. We provide services to over 400 individuals yearly.

Our accomplishments are shown every three years with the renewal of the CARF (Commission for Accreditation of Rehabilitation Facilities) accreditation process. Our programs are all outcome based. Every program has overall goals and each program has participant goals. Every participant has an individual service plan addressing major concerns in their lives: physical therapy, occupational therapy, social skills, work skill development, recreational, medical, daily living skills, employment, and residential goals. The outcome goals must meet CARF standards. Headwaters, Inc. has been CARF accredited since 1990.
